Greene County Board of Developmental Disabilities invites you to join their 2017 Spring Bowling League! GCBDD has contracted with Toward Independence (TI) for staffing and execution of the bowling leagues. Individuals are responsible for their own transportation to and from bowling. TI staff cannot transport anyone home. Please be prompt in arriving at the bowling alley for pick up. If a participant uses a wheelchair or a walker, they MUST have someone there to assist them due to liability issues.
This form will also serve as our emergency contact information, so please fill out the form in its entirety. You may register multiple bowlers by listing each person’s name on the back of the registration form. Once you have completed the required information, detach the Bowling Registration Form and submit it no later than May 8, 2017.
GCBDD will provide pizza on the last night (June 13th) after bowling is done! Please bring money for beverages. There will also be an informal awards ceremony. Medals and certificates will be given out to the participants.
