Greene County Board of Developmental Disabilities invites you to join
their 2018 Winter Bowling League!
When: 11:30a.m. to 1:00p.m. on Saturdays – January 6th, 13th, 20th, and 27th
Where: Beaver Vu Bowl – 1238 N. Fairfield Rd. Beavercreek, OH 45432
Cost: $28.00 per bowler (covers rental of ball, lane, shoes, and the cost of two games each day for 4 weeks. Limited to the first 32 bowlers who register.
Payment: Checks can be made payable to ‘Beaver Vu Bowl’.
Deadline: Registration and payment must be received by Friday, January 5th.

Individuals are responsible for their own transportation to and from bowling. Please be prompt in arriving at the bowling alley for bowling and for pick up. If a participant needs assistance of any kind, they MUST have someone (family member, staff, etc.) there to assist them due to liability issues.
This form will also serve as our emergency contact information, so please fill out the form in its entirety. You may register multiple bowlers by listing each person’s name on the back of the registration form. Once you have completed the required information, detach the Bowling Registration Form and submit it no later than Friday, January 5, 2018.
GCBDD will provide pizza on the last day (January 27th) after bowling is done! Please bring money for beverages.Each bowler will be given a medal and a certificate after bowling is completed on January 27th.
